Senior Referral Services
There are a confusing number of services for older individuals, but HCM is here to help. Our services are free; there is no charge for our assistance. We do our best to connect you to the right agency or organization to solve your concern. HCM can also assist in filling out applications for various services. Our goal is to promote independence, activity, and community interaction, so give us a call at (502) 451-3695 ext. 260 if you need help with any of the following:
- Mental health services
- Eligibility for community programs
- Housing assistance (HUD, Section 8, long-term care, assisted living, and personal care)
- Acquiring health aids including walkers, canes, and wheelchairs
- Guardianship issues
- Social Security
- Medicare, Medicaid and Medicare Part D
- Billing concerns
HCM can also provide application assistance with:
- TARC3 – The ADA states that TARC’s regular bus service should be the primary means of public transportation for everyone, including people with disabilities. Under ADA, TARC3 will serve those persons who do not have the functional capability to ride TARC’s regular buses.
- LIHEAP (Low Income Home Energy Assistance Program) – The LIHEAP program helps keep residents safe and healthy through initiatives that assist with energy costs.
- QMB (Qualified Medicare Beneficiary) – The QMB Program is one of the four Medicare Savings Programs that allows you to get help from your state to pay your Medicare premiums. This program helps pay for Part A premiums, Part B premiums, and deductibles, coinsurance, and copayments.
- Food Assistance – There are several programs available to make nutritious food available to seniors in our area. Let HCM help to see if you qualify for any of these beneficial programs.
